Insurance
Must be uninsured or underinsured. Medicare Part D patients may qualify if they have spent a certain amount on medications in the current year and meet income requirements. Patients with commercial insurance are generally directed to the savings card.

Must be a resident of the United States, Puerto Rico, or a U.S. territory.
