Income
Up to $79,800 for a single person
Insurance
Must be uninsured or have Medicare Part D coverage and not be eligible for Low Income Subsidy (LIS); must not have other insurance coverage for the medication.
Residency
Must be a resident of the United States, Puerto Rico, or U.S. Virgin Islands.
Income Limits (500% FPL)
The program guideline sets the limit at 500% of the federal poverty level. Figures use the 2026 HHS guidelines for the 48 contiguous states.
